Student Records
Two categories of records
1. Directory Information
a. Available to anyone, unless the student says no
b. Examples
- Name
- Local and permanent address
- Electronic mail address
- Telephone number
- Date of birth
- Current semester schedule of classes
- Dates of attendance and graduation
- Degrees and awards received
- Academic level
- Major field of study
- Participation in officially recognized activities/sports
- Weight/height of members of athletic teams
2. Personal Information
a. Except for "need to know", not available without consent of student
b. Examples
- Course grade
- Transcript
- Information on family members
- Social security number
- Financial aid received
- Earnings from campus employment
- Donations to USU
- Disciplinary action, including parking tickets
Policies for Providing Information About Students
1. Students may control the release of directory information by completing
a. Privacy Hold form at the Registrar's Office.
2. Phone Requests
a. Releasable information may be provided over the phone.
b. Nonreleasable information or information protected by a Privacy Hold may not be released over the phone, unless student signs a release.
3. Written Requests
Student must sign all requests for written verification of his or her records.
4. Parents
Parents can receive all information about their unmarried children if:
a. They claim the child as a dependent on their federal income tax
form.
b. They are providing financial assistance.
